- Your Manage My Case (MMC) Username and Password
- Your Doctor’s name (your Primary Care Provider or PCP)
- Your Doctor’s phone number
- Your Pharmacy name
- Your Pharmacy’s phone number
- Your Health Plan name
- Your Health Plan’s phone number
- Your Health Plan Member ID Card
