To check on your benefits, it is important to know your identification numbers and username and passwords. This information is necessary to access your accounts and when you need to renew your coverage.
You should keep a list, copy, or picture of the following information somewhere safe:
- Your Medical Card
- Your Manage My Case (MMC) Username and Password
- Your Doctor’s name (your Primary Care Provider or PCP)
- Your Doctor’s phone number
- Your Pharmacy name
- Your Pharmacy’s phone number
- Your Health Plan name
- Your Health Plan’s phone number
- Your Health Plan Member ID Card
